Do I Need a GP Referral to See a Physio in NZ? ACC and Physiotherapy Explained

If you've tweaked your back, rolled an ankle, or picked up a niggle at work, one of the first questions people ask is: "Do I need to see my doctor before I can see a physio?" It's a fair question, since in a lot of healthcare systems, you do need a referral to see a specialist.

Good news: in New Zealand, physiotherapy doesn't work that way.

At Massey Waitakere Physiotherapy, we see people walk in every week who've been putting off getting help because they assumed they needed a doctor's letter first. So let's clear that up, along with how ACC fits into the picture if your injury happened suddenly.

Can I See a Physio Without a GP Referral?

Yes. In New Zealand, physiotherapists are registered health professionals you can see directly, with no referral required. This is sometimes called "self-referral." You can book an appointment at our Massey or Henderson clinic the same way you'd book a hairdresser or a dentist: just get in touch and choose a time that suits.

This applies whether you're paying for physiotherapy privately or your injury is covered by ACC (more on that below). A referral from your doctor is not required to walk through our door, though if your doctor has recommended physio as part of a wider treatment plan, we're always happy to work alongside them.

What Is ACC and How Does It Cover Physio?

ACC (the Accident Compensation Corporation) is New Zealand's no-fault accidental injury scheme. If you've been injured, whether that's a sprained ankle playing weekend sport, a strained back from lifting at work, or a fall around the house, ACC may cover a portion of your physiotherapy costs.

Here's the part that surprises a lot of people: your physiotherapist can lodge the ACC claim for you at your very first appointment. You don't need to see a GP first, fill out separate paperwork on your own, or wait for approval before treatment starts. We'll talk you through your injury, confirm it's ACC-eligible, and get the ACC claim for physiotherapy underway so you can begin your rehab without delay.

Conditions that build up gradually rather than from a specific accident (like general wear-and-tear back pain) usually aren't ACC-covered, since ACC is specifically for accidental injuries. If that's your situation, we'll let you know upfront and talk through private treatment options instead.

What Does ACC Physiotherapy Cost?

ACC covers a significant portion of the cost of your physiotherapy visits, but it doesn't cover 100%: a standard consultation surcharge usually applies, which you pay at each visit.

At Massey Waitakere Physiotherapy, our current ACC surcharges are:

  • Standard rate: $37 for your initial visit, $32 for follow-up visits
  • Gold Card holders: $33.50 initial, $29 follow-up
  • Under 12s: $33.50 initial, $29 follow-up
  • Community Services Card holders: $15 initial, $15 follow-up

If your injury isn't ACC-eligible and you're paying privately, our standard rates are $85 for an initial consultation and $70 for follow-ups (with reduced rates for Gold Card, Community Services Card, and under-12 patients).

What Should I Bring to My First Appointment?

You don't need anything special, though it helps to bring:

  • Any ACC claim number if you've already started a claim (e.g. through an urgent care clinic or GP)
  • A list of current medications
  • Loose or comfortable clothing that allows access to the injured area
  • Any relevant scan or specialist reports, if you have them

When Should I See a Doctor Instead of a Physio First?

Physiotherapists are well trained to recognise when something falls outside their scope and needs urgent medical attention. That said, if you're experiencing symptoms like severe unexplained pain, loss of feeling or function, signs of a fracture, chest pain, or any injury involving significant trauma, please see a doctor or head to an emergency department first rather than waiting for a physio appointment.

For the vast majority of everyday sprains, strains, post-surgery rehab, and nagging aches, physiotherapy is a great first port of call, and often the fastest way to get moving again.
